Community Services Department
MISSION:
The Community Services Department of Southwestern Virginia Mental Health Institute is an integral part of service delivery in the psychiatric service continuum in Southwest Virginia. The Department is responsible for providing a direct link between the internal and external customers of the Institute and for monitoring and identifying deficiencies in the linkage system. Processes and procedures are assessed and modified to optimize cooperation and collaboration between Institute staff and in-patient and out-patient care providers.
Directly linked to the facility strategic plan, goals of the department are met by monitoring and mediating individual case problems; identifying systemic issues by regularly scheduling meetings between the Institute and Community Services Boards' staff; through effectiveness of discharge and customer satisfaction surveys; and involvement with state, regional, and local issues influencing continuity of care for the mentally ill citizen of Southwest Virginia.
PARTNERSHIPS IN CONTINUITY OF CARE
The philosophy behind continuity of care revolves around the knowledge that multi-agency partnerships are needed to maintain a support network for mentally ill citizens of Southwest Virginia. These partnerships are developed from needs based discharge planning and depend upon the collaboration and cooperation of SWVMHI staff and the SWVA Community Services Boards. The Southwest Virginia region is known state wide for their cooperative partnerships.
OUTREACH
The Community Services Department at Southwestern Virginia Mental Health Institute actively participates in the following regional activities:
1. Regional conferences on mental health issues
2. Mt. Rogers Community Resource Team
3 Liaison meetings and trainings
4. New River Valley Discharge Planning Committee
5. Chair for the Annual Family Day Chair
